  PRN, Day shift

• Support patients, families, and loved ones during their end-of-life journey.
• Perform comprehensive assessments to establish goals for symptom management and end-of-life care.
• Implement effective case management strategies.
• Collaborate with an interdisciplinary team to address patients’ needs holistically.
• Develop and maintain therapeutic relationships through effective communication.
• Utilize state-of-the-art technology to document skilled services provided.

• Education: Registered Nurse with Valid CT RN license
• Experience: One year of RN experience;
  Homecare or Hospice experience preferred
• Valid driver’s license and reliable transportation required
• Computer proficiency required

• Automatic enrollment into the Nurse Residency Program for graduate RNs with less than one year of experience
