About This Role

The Hospice Nurse Weekend (RN) provides skilled nursing care to hospice clients in their homes or community health settings, ensuring high-quality, cost-effective care. This role is ideal for nurses who are passionate about making a difference in the lives of patients and their families during critical moments.

• Location: Norwich, CT
• Department/Unit: 95005-Norwich Hospice Gen
• Schedule: Part Time, Weekend shift

• Provide skilled nursing care to hospice clients in home or community settings.
• Conduct complete nursing assessments based on referral data and clinical observations.
• Develop and modify plans of care in collaboration with clients and interdisciplinary teams.
• Facilitate peaceful death and assure appropriate bereavement follow-up.
• Utilize Hospice Levels of care and follow IDT processes for medication and treatment approvals.
• Maintain client records according to agency standards.
• Make follow-up visits to assess disease progress and response to treatment.
• Teach and support clients and families regarding health and end-of-life issues.
• Collaborate with physicians and direct LPNs in executing care plans.
• Participate in client care conferences and in-service education programs.

• Associate’s degree or diploma in nursing required
• BSN preferred

• One year of nursing experience required
• Experience in medical/surgery or home care preferred

• Current RN License in Connecticut required
