JOB SUMMARY
The Family Resource Coordinator will work under the supervision of the Director of Family Bridge-Eastern CT. The Family Resource Coordinator is a community‑based position whose responsibilities include but are not limited to, scheduling Integrated Home Visits (IHV) for family’s bedside in the hospital and via phone, working alongside Registered Nurse Home Visitors, evaluating needs of clients, referring clients to community resources, coordinating appointments, providing help with enrollments with diverse resources in the community.
The Family Resource Coordinator assists families in their communications with medical offices, social service agencies, DCF and other organizations. All work with families is short term with the objective of increasing their ability to self‑advocate.
