The New London Homeless Hospitality Center (NLHHC) seeks a Rapid Re-Housing Case Manager to help individuals move from homelessness into permanent housing and build the stability and connections needed to remain housed.

The New London Homeless Hospitality Center (NLHHC) seeks a Rapid Re-Housing Case Manager to help individuals move from homelessness into permanent housing and build the stability and connections needed to remain housed.

The Role:

From Homelessness to Housing Stability

Help individuals who have experienced homelessness successfully transition into permanent housing. Using a Critical Time Intervention (CTI) approach, you’ll provide focused, practical support during the critical months after someone becomes housed, helping them strengthen the skills, resources, and community connections they need for long-term stability.

What You’ll Do
  • Build individualized housing stability and CTI plans
  • Support individuals as they transition into permanent housing
  • Help participants understand tenant responsibilities for successful tenancy
  • Connecting participants to community resources
  • Identify barriers early and problem-solve before they become housing crises
  • Mediate concerns between tenants and landlords when needed
  • Help participants develop budgets and strengthen independent living skills
  • Coordinate with landlords, property managers, and community partners
  • Gradually reduce case management support as participants build independence and long-term support systems

  • Excellent problem-solving and follow-through
  • Ability to manage a caseload and stay organized
  • Comfort working independently while being an active part of a collaborative team
  • Ability to respond calmly and thoughtfully to challenging situations
  • Strong community partnership skills
Bonus:

Experience in housing, homelessness services, case management, behavioral health, benefits, employment services, or other human services.

What Success Looks Like

You’ll build trust quickly, help participants navigate barriers to housing stability, and develop strong relationships with community partners. Over time, you’ll manage your caseload independently, proactively address concerns, and help participants build the supports they need to remain successfully housed.
