Job Description SummaryThe Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of the Option Care pharmacy practice professional standards and applicable regulatory entities. This entry‑level position introduces the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use.
Job Responsibilities- Determine the suitability of individual patients for home care, collecting and organizing all patient‑specific information needed, and assessing suitability in accordance with Option Care policies, medical condition and prescribed medication therapy, and prognosis with clearly defined outcome goals.
- Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for the pharmacotherapy of home care patients; make suitable recommendations for medication modifications or monitoring parameters; identify appropriate monitoring parameters; complete nutritional assessment and make appropriate recommendations.
- Modify pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient‑specific information; demonstrate competence in pharmacokinetic principles, formulas and laboratory value assessment.
- Provide medication‑use education to Option Care staff, other professionals, home care patients and/or their caregivers using effective patient education techniques.
- Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings, communicate pertinent patient information systematically, prepare and dispense medications per Option Care policies, calibrate equipment accurately, prepare medications to appropriate concentration, and maintain safety and quality assurance practices.
- Manage the use, maintenance and troubleshooting of medication administration equipment and medication‑related equipment; program, maintain and advise others on use; participate in incident and equipment error reporting; complete QAR documents and monitor inventory discrepancies.
- Provide concise and timely responses to drug information requests from health care providers and patients; formulate responses based on literature analysis; attend educational seminars or CEU opportunities to promote professional competency and comply with state mandated CEU requirements.
- Manage the use of investigational drug products according to established regulations and Option Care policies; utilize pharmacy support personnel effectively; prioritize workload, organize workflow and ensure accuracy of support staff work.
