BAYADA Home Health Care is seeking a Home Health Physical Therapist for a position in Norwood, Massachusetts.

Key responsibilities include:

• Making visits to clients in their homes
• Performing diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing and safety assessments
• Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance and other interventions
• Continually assessing and revising the Physical Therapy care plan and participating with nursing in the multidisciplinary care plan, as appropriate
• Educating and instructing clients, family members or other client representatives in rehabilitative care and activities necessary to promote the client’s health, safety and independent living
• Accurately documenting observations, interventions and evaluations pertaining to client care management and services provided, utilizing a touchpad tablet

Qualifications include:

• A current license as a Physical Therapist in Massachusetts

Base Pay: $65.00 - $75.00 per point, depending on qualifications, plus a generous Weekend Differential.
