Lifeworks is hiring a Relief Case Manager to work with Individuals with /DD in one of our Residential Programs. We are a nonprofit human service agency that provides adults with developmental disabilities with the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve.
Candidates must have related or direct experience working with the community we serve.
- Provide care and assistance to individuals who live independently in the community. Assist individuals with tasks such as banking, grocery shopping, paying bills, transportation to healthcare appointments, and participating in a variety of one‑on‑one and group social activities.
- Provide transportation to individuals as necessary.
- Other duties and/or assignments to work/program locations based on the needs of the person served and Lifeworks.
- Experience working with Individuals with /DD.
- High School diploma/GED, related experience preferred.
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks.
- Valid Driver's License required.
- Excellent verbal and written communication skills.
