Position: Patient Care Manager (Regional Floater)

Are you a seasoned Home Health RN with a talent for leadership and a desire for a change of scenery? Our client is looking for a Regional Floater Patient Care Manager (PCM). Based out of Norwood, MA, you will act as the "Clinical Elite" for the region, stepping into different branches to provide management coverage, mentor new staff, and stabilize clinical operations where needed most.

This is not a "desk job"—it is a high-level leadership role for someone who loves the puzzle of home health management and enjoys moving between different teams and territories.

Key Responsibilities
  • Interim Leadership: Serve as the acting Patient Care Manager for various branches during vacancies, leaves of absence, or periods of high growth.
  • Clinical Auditing: Conduct "deep dive" OASIS reviews and clinical chart audits to ensure branches remain in high standing with CMS and state regulations.
  • Staff Mentorship: Onboard and train new field clinicians and incoming PCMs on agency protocols and documentation standards.
  • Utilization Review: Analyze patient care data to ensure appropriate visit frequencies and resource allocation across the region.
  • Troubleshooting: Identify operational bottlenecks in different branches and implement corrective action plans to improve efficiency and patient outcomes.
Qualifications
  • Licensure: Active, unencumbered Registered Nurse (RN) license in the state of Massachusetts.
  • Experience: Minimum of 2–3 years of recent Home Health experience is required.
  • Leadership History: Proven experience in a supervisory role (PCM, Clinical Supervisor, or Team Lead) within a Medicare-certified home health agency.
  • Expertise: Mastery of OASIS-E and a strong grasp of the PDGM (Patient-Driven Groupings Model) reimbursement structure.
  • Mobility: Must have a valid drivers license and be willing to travel to various branch locations within the assigned region (mileage reimbursed).
