Job DescriptionPosition Overview:
We are seeking a dedicated and experienced Registered Nurse (RN) to join our team as a Patient Care Manager in the Home Health (HH) Regional Floater role. This dynamic position requires an individual who is adaptable, compassionate, and skilled in managing patient care across various locations within the region. The ideal candidate will excel in coordinating and overseeing patient care plans, ensuring high‑quality service, and supporting clinical staff in delivering exceptional care.
Responsibilities- Manage patient care across multiple locations
- Ensure compliance with health regulations and standards
- Provide guidance and support to nursing staff
- Conduct assessments and develop care plans
- Foster relationships with patients and their families
- Registered Nurse with active state licensure
- 2+ years in a supervisory role in home care setting preferred
- In-depth knowledge of Medicare and regulatory practices
- Experience with OASIS documentation
- Track record of working in a collaborative, team‑oriented environment
