Patient Care Manager – Fall River

Schedule: Full-time | Monday – Friday | 8 a.m. to 5 p.m.

• Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements.
• Coordinate and monitor patient care delivery.
• Supervise clinical staff to ensure consistent, appropriate service.
• Maintain clear communication with patients, caregivers, referral sources, and field and office staff.
• Drive quality outcomes by meeting department goals and supporting improvement initiatives.
• Model professionalism and organizational values in daily work.
• Uphold compliance through required training, accurate reporting, and cooperation with audits.
• Promote a safe work environment by following established safety protocols.
• Support staff growth through coaching and continuing education.

• Registered nurse with current licensure to practice nursing in the practicing state.
• Previous experience in a home care setting with two years of management or supervisory experience, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements.
