Patient Care Manager – Fall River
Schedule: Full-time | Monday – Friday | 8 a.m. to 5 p.m.
Key Responsibilities- Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements.
- Coordinate and monitor patient care delivery.
- Supervise clinical staff to ensure consistent, appropriate service.
- Maintain clear communication with patients, caregivers, referral sources, and field and office staff.
- Drive quality outcomes by meeting department goals and supporting improvement initiatives.
- Model professionalism and organizational values in daily work.
- Uphold compliance through required training, accurate reporting, and cooperation with audits.
- Promote a safe work environment by following established safety protocols.
- Support staff growth through coaching and continuing education.
- Registered nurse with current licensure to practice nursing in the practicing state.
- Previous experience in a home care setting with two years of management or supervisory experience, preferred.
- Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements.
- Strong job stability & demand.

Annual Salary: $90,000 – $100,000 USD.
