Job : 502

Location: Norwood, MA

Shift: Sunday-Thursday, 10:00am-6:00pm

Salary: $50,000

Responsibilities
  • Provide care and assistance to individuals who live independently in the community. Assist individuals with tasks such as banking, grocery shopping, budgeting, paying bills, planning vacations, managing healthcare, and coordinating and participating in a variety of one-on-one and group social activities.
  • Promote optimal health for individuals through routine, preventive and emergency health care. Ensure that residential environments are safe, secure and free from harm.
  • Provide education, training, counseling, and physical assistance when necessary to achieve ISP goals in functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication skills, community mobility, self-esteem, and money management.
  • Develop, write and present annual ISPs and semi-annual assessments in consultation with other residential staff and a Director.
  • Provide transportation as necessary.
  • Communicate with persons significant to the individual served (family, friends, guardians, funding sources, advocates, volunteers, employers) to facilitate consistency of programming and support.
  • Perform case-management duties including maintenance of confidential files, ensuring cases are reviewed, and documenting activities and training.
  • Attend regular meetings as necessary.
  • Other duties and/or assignments to work/program locations based on the needs of the company.
Requirements
  • Experience working with individuals with /DD.
  • Bachelor’s degree from a four-year college or university; related experience preferred.
  • Must be able to pass fingerprint background check, CORI, DPPC, and driving background checks.
  • Valid Driver’s License required.
  • Excellent verbal and written communication skills.
