Clinic Assistant Scheduler/Licensed Practical Nurse – PACE of Cincinnati
Tri Health is actively recruiting for a Clinic Assistant Scheduler- LPN to join the PACE of Cincinnati PACE Center located in Norwood Ohio. The Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.

Location- PACE of Cincinnati – 4850 Smith Rd, Cincinnati, OH 45212 (requires driving around the Greater Cincinnati Area)

Work Schedule- 80 hours bi-weekly
- Graduate of an approved technical, professional, or vocational program in Nursing (Required).
- 1-2 years of experience working with a frail or elderly population or training from PACE on working with a frail or elderly population before providing direct patient care (Required).
- 3-4 years of LPN clinical experience (Preferred).
- Current LPN license in good standing to practice in Ohio (Required upon hire).
- Basic Life Support (BLS) certification (Required).
The PACE LPN provides general nursing care to PACE participants, executes nursing interventions under the supervision of the PACE primary care providers and RN nursing leadership staff, and collects data on participant medical conditions in the PACE clinic and by telephone. The PACE LPN provides nursing education to participants, caregivers, and PACE staff, and is responsible for scheduling participant appointments for PACE Clinic referrals to medical specialists and ancillary care providers in the community.
Job Responsibilities- Under the supervision of the primary care provider and PACE nursing leadership staff, provide nursing interventions and complete PACE participant nursing interventions in the PACE Clinic or community setting, and escalates any nursing concerns to the primary care provider and PACE nursing leadership.
- Serve as a nursing subject matter expert and communicate best practices for a frail elderly population to the IDT, participants, and caregivers.
- Schedule participant appointments for referrals to medical specialists and ancillary care providers in the community; enter instructions in the EHR for these appointments.
- Document nursing interventions in participant care plans and the PACE EHR.
- Administer participant medications in the PACE Clinic as directed by the primary care provider and PACE nursing leadership staff.
- Review, sign off, and disseminate nursing orders and instructions to appropriate PACE staff and PACE contracted personnel.
- Provide care coordination during care transitions and proactively communicate with hospital and long-term care facility case managers regarding participant discharge care plans.
- Monitor PACE participants for changes in their health or psychosocial status and promptly communicate pertinent changes to the IDT.
- Attend IDT and morning meetings and report on participants’ clinical status as directed by PACE nursing leadership staff.
- Participate in the nursing on-call rotation to provide after-hours and weekend call coverage to PACE participants.
