Therapy / Rehabilitation - SLP Home Health

Location Radius for Home Health Patients: All Marin County, 50 miles
Certification Requirements

The Speech Therapist is responsible for developing and implementing rehabilitation home therapeutic treatment plans. This includes educating patients, family members, and significant others in home health and hospice settings. The role requires coordination with other home health staff to ensure efficient integration of care.
Job AccountabilitiesRehabilitation Services
- Develop or implement treatment plans for issues such as stuttering, delayed language, and swallowing disorders.
- Complete patient evaluations and tests per Speech Pathology Standards of Practice and other required standards.
- Develop and update the patient's Plan of Care (POC).
- Assess and provide or progress treatment for patients per the plan of care according to established standards.
- Complete all documentation related to charge capture per established standards.
- Coordinate patient care effectively and efficiently with other care providers and support team members.
- Ensure compliance with department and organization requirements and responsibilities.
- Work flexible shift assignments, including days of the week and hours.
- Assist with clinical supervision of assistants and aides.
- Function as a clinical instructor for assigned students.
- Prepare and complete accurate documentation related to patient care, including all therapy performed in medical and departmental records per required standards.
- May perform general clerical tasks such as answering phones, updating patient appointments, or managing Electronic Health Records (EHR).
- Participate in quality improvement processes and interdisciplinary projects.
- Maintain a clean, neat, and safe working environment, including cleaning and disinfecting equipment after each use.
- Inspect and use equipment per manufacturers' guidelines, recording data and reporting malfunctions per policy.
- Maintain the strictest confidence of all patient Protected Health Information (PHI) and protect all PHI from accidental, intentional, or inappropriate disclosure.
- Prepare, administer, and document services in accordance with state and federal regulations.
- Comply with department and organization requirements and responsibilities.
- Retain certification and complete mandated continuing education requirements to meet facility and regulatory agencies' accreditation and licensure requirements.
Base pay range: $56.90/hr - $58.60/hr
