Job Coach, Social Work, Career & Life Skills Coaching

Sevita, a leader in home and community-based health care, is seeking a Job Coach to support individuals with diverse abilities in achieving meaningful employment. In this role, you'll provide person-centered coaching, from assessing strengths and interests to teaching job search, workplace, and daily living skills. You'll collaborate with care teams, families, and employers to create inclusive work opportunities while tracking progress and outcomes.

Responsibilities

• Provide one-on-one job coaching to individuals with diverse abilities to build work readiness and independence
• Assess vocational strengths, interests, and support needs to develop individualized employment plans
• Teach job search skills including resume writing, applications, and interview preparation
• Offer on-the-job support such as task modeling, cueing, and workplace social skills coaching
• Collaborate with families, caregivers, employers, and care teams to coordinate services
• Document progress, maintain accurate case notes, and report outcomes
• Advocate for workplace accommodations and inclusive employment practices
• Facilitate small group workshops on employment and life skills
