Job Summary

Diabetes Educator – BJC Health Care, O'Fallon, Missouri. Full‑time, day shift, Monday through Friday with no weekends, holidays, or on‑call duties. Salary range $62,400.00 – $ per year, commensurate with qualifications and experience.

Provides diabetes and nutritional educational services and community resources for inpatients and outpatients. Maintains a case load of patients and promotes program development.

• Monitors patients’ response and modifies plan of care accordingly.
• Provides accurate documentation as required.
• Delivers direct patient care by assessing and evaluating patients’ physical, psychosocial, and emotional needs.
• Develops and implements individual plans of care in accordance with established guidelines and standards of nursing care.
• Educates patients and their families on how to manage their condition.

• Education:
  
  Nursing Diploma or Associate’s degree – Nursing/Nutrition/related.
• Experience:
  
  Less than 2 years.

  Registered Nurse (RN).

• Education:
  
  Bachelor’s degree in Nursing.
