Job Summary

The primary purpose of your position is to deliver patient care in accordance with applicable Federal, State, and local standards, guidelines, and regulations, as directed by the Administrator, ensuring the highest quality of care and safety.

Supervises: LPNs, CNAs, Medical Assistants.

Reports to:

Nurse Manager or Practice Manager.

Essential Functions
  • Clinical Care

    Embodies the mission, vision, and values of Martha’s Vineyard Hospital and the Primary Care Department. Applies knowledge of illnesses, injuries, and diseases and uses available resources in the assessment process. Completes nurse visits as assigned, documents patient assessment, nursing interventions, and their effectiveness. Implements clinical and technical aspects of care and nursing orders in accordance with established policies and procedures. Intervenes with proper technique, procedures, and safety precautions to meet individual patient needs.

    Manages patient care within the department, coordinates with providers, practice managers, PSCs, department coordinators, and MAs, and delegates tasks as needed. Conducts AED/O2 checks in the department. Manages incoming clinical needs faxes, routes from front desk, separates provider faxes, and distributes them. Coordinates care with outside agencies, consults with in‑house resource personnel, and respects patient dignity and confidentiality while acting as an advocate.

  • Patient and Family Instruction/Education

    Reviews patient and family knowledge of diseases and readiness to accept care ntifies needs for care or alternatives such as outpatient treatment. Sets realistic learning objectives by including patients and families in instruction planning. Provides self‑care instructions and communicates necessary medical information to community resources. Uses individualized techniques to meet specific patient needs and initiates referrals when needed.

  • Quality of Work

    Adheres to the nursing dress code and wears an identification badge. Demonstrates organizational skills while adapting to changing workloads. Performs tasks per policy, including participation in the plan of care with the entire team, medication/immunization administration, nursing procedures, and patient education. Maintains open communication channels with colleagues and physicians and communicates factual information in reports.

  • Personal Growth and Development

    Identifies personal learning needs, seeks opportunities for self‑growth, shares skills gained from continuing education classes, and maintains current BLS competency.

  • Age‑Specific Competency

    Evaluates patients for age‑appropriate behavior, motor skills, and physiological norms. Recognizes psychological development and adapts approaches accordingly. Anticipates lifestyle changes that may affect the care plan, develops strategies, and evaluates support systems. Uses age‑appropriate interview and communication techniques and assesses patients for signs and symptoms of abuse or neglect, referring to appropriate personnel or community agencies.

  • Risk Exposure Categories

    The role may involve exposure to blood or body fluids or may include performing Category 1 tasks, or may have no risk of exposure.

  • Working Conditions

    Sits, stands, bends, lifts, and moves intermittently during working hours. Is subject to frequent interruptions, communicates with the medical staff and other departments, and may be exposed to infectious waste and diseases. Must be constantly alert for patient safety.

Qualifications
  • Education – Bachelor’s Degree in Nursing or Associate’s Degree in Nursing required. Experience cannot be used in lieu of a degree.
  • Licenses & Credentials – Registered Nurse (RN) with a current State license;
    Basic Life Support (BLS) required.
  • Experience – Minimum one year as an RN; 0‑1 year preferred. Physician office experience preferred. New graduates will be considered.
