Ready to join? We are looking a part-time Outpatient Physical Therapist to join our outpatient facility team. This position will be based at the Oak Brook, IL facility. The position will provide approximately 20 hours per week. The Physical Therapist is responsible for the evaluation, development and implementation of treatment programs in collaboration with physician orders.
Essential Duties and Responsibilities- Evaluates patient and gathers appropriate information to determine accurate clinical diagnoses and treatment plan.
- Establishes appropriate, functional and objective goals based upon clinical examination.
- Devise detailed treatment and care plans for patients.
- Provides exceptional patient care and service.
- The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships.
- Cultivate relationships with patients and the community.
- Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care.
- Maintain patient charts, clinical logs and checklists necessary for compliance.
- Maintains patient confidentiality with regard to medical records per company policies and procedures and assures compliance with all HIPAA guidelines.
- Communicates well in a professional setting.
- Perform additional duties as required.
- Bachelor’s, Masters or Doctorate Degree from an accredited Physical Therapy Program
- Current State of Illinois Physical Therapy license
- CPR Certified
