Physical Therapist – Home Health
Location: Calumet City, South Holland, Dolton, Riverdale, Harvey, Dixmoor, Phoenix, Posen, Burnham, Hegewisch, Thornton, Lansing, Glenwood, Lynwood
Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
Rotating weekends required + 1 summer holiday & 1 winter holiday
Bonus: Up to $10,000 sign‑on bonus for eligible external candidates (must be in role at time of payout; not available to current team members)
What You’ll DoAs a Home Health Physical Therapist, you will deliver high-quality, patient-centered care in a home setting, helping patients improve mobility, function, and quality of life. You’ll collaborate with an interdisciplinary team while working independently across your assigned territory.
Key Responsibilities- Evaluate, develop, and implement individualized treatment plans based on patient needs and evidence-based practices
- Monitor patient progress and adjust care plans to achieve optimal outcomes
- Provide clear, timely, and compliant documentation to support care and reimbursement
- Educate patients and families on treatment plans, self-care, and safety practices
- Coordinate care with physicians, nurses, and other healthcare team members
- Participate in case conferences, team meetings, and quality improvement initiatives
- Support safe patient handling and promote fall and injury prevention
- Strong clinical judgment and ability to manage a diverse patient population, including complex cases
- Excellent communication and collaboration skills
- Ability to work independently and manage time effectively in a field-based role
- Commitment to delivering compassionate, patient-focused care
- Licensed Physical Therapist in Illinois (or eligible for licensure)
- Minimum 1 year of clinical experience (or strong clinical affiliation experience)
- Basic computer skills (documentation systems, email, etc.)
- Valid driver’s license and reliable transportation
- Travel required within assigned service territory
- Work in patient homes and assisted living settings
- Ability to lift up to 35 lbs (with assistance/equipment as needed)
- Frequent standing, walking, bending, and assisting patients with mobility and transfers
- Autonomy in your daily schedule with strong team support
