Home Care RN​/Palliative- RN

Position Purpose

The Palliative Care RN is responsible for patients and families in the palliative care program and provides palliative care clinical consultation.

Full time

• Collaborate with manager to develop performance measurements for the Palliative Care Program to ensure continuous quality improvement and ensure implementation.
• Motivate and support employees to achieve exemplary performance on internal and external customer service, productivity, and accuracy.
• Provide statistical and budgetary data to the appropriate Palliative Care program.
• Collaborate with all departments to ensure quality delivery of services.

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in the state of practice.
• CPR certification.
• Hospice/Palliative Care certification preferred.
• Home care experience, strong clinical knowledge base, hospice experience, computer skills, and strong interpersonal and organizational skills.
• Palliative Care experience; demonstrate ability to use problem solving, critical thinking, and priority setting skills.
• Must be able to work independently and on a team.
• Ability to perform diversified duties with time limitations.
• Ability to develop, coordinate, and evaluate programs in Palliative care and end-of-life care.
