The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises nursing assistants to ensure high‑quality care in compliance with federal, state, and local regulations. Key responsibilities include planning and overseeing daily nursing activities, administering medications, completing required documentation, and maintaining patient confidentiality. The LPN also performs professional nursing services such as catheterization, wound care and IV care while fostering a warm and collaborative environment.
In addition, the role involves coordinating with physicians, ensuring that safety protocols are followed and maintaining effective communication with residents, families, and staff to meet residents’ needs.
- Performs all functions under your active license as it pertains to your assigned duties.
- Knows the residents you serve, providing care based on their care plans and physician orders.
- Prepares and administers medications as ordered by the physician.
- Develops and maintains good working rapport with inter‑departmental personnel, ensuring that nursing services and activities can be adequately maintained to meet the needs of the residents. Creates and maintains an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. Receives/gives the nursing report upon reporting in and ending shift duty hours.
Makes rounds with physicians as necessary. - Assists in all aspects of a resident’s daily life, including activities of daily living, meal service and social engagement.
- Communicates appropriately with clinical leadership when a resident’s needs change.
- Know how to approach and care for residents with dementia.
- Is willing to take on other duties and assignments within the scope of knowledge and experience.

- Licensed Practical Nurse (LPN) with a current unencumbered state license.
- Knowledgeable of nursing & medical practices & procedures, laws, regulations and guidelines that pertain to nursing care facilities.
- Experience in Long Term Care is preferred.
- Willing to take additional training or education as required by specific states.
- CPR and First Aid certifications are required.
- Ability to be flexible and adaptable while handling multiple priorities.
- Possesses written and verbal skills for effective communication.
- Competent in organization and time management skills.
- Demonstrates good judgment, problem‑solving and decision‑making skills.
- Required to sit, stand, bend and walk intermittently throughout the workday; lift and/or move up to 100 lbs.
- Must be able to cope with the mental & emotional stress of the position.
- May include health requirements set forth by the policies of this facility, which include a medical & physical examination.
- May be necessary to assist in the evacuation of residents during emergency situations.
- Visual and auditory ability sufficient for written and verbal communication.
- The noise level in the work environment is usually moderate.
