Full time Home Health Registered Nurse Case Manager for South East Chicago. Normal office hours are Monday through Friday 8:00am to 5:00pm. Position includes weekend rotation availability every 4th weekend and 2 holidays per year for coverage. Rotating after hours on call coverage for PRN patient concerns equitably distributed across area nursing team.

This position is eligible for up to a $10,000 sign‑on bonus. You must be actively working in this same position at the time of payout. Current internal team members are not eligible at this time.
