LPN – Home Health | Southeast City

Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
Rotating holiday schedule (2 per year)

Join our Home Health team as a Licensed Practical Nurse (LPN), delivering compassionate, high-quality care to patients in the comfort of their homes. Working under the supervision of an RN, you’ll play a key role in supporting patient outcomes, coordinating care, and empowering patients and families through education.
Key Responsibilities- Deliver skilled nursing care in accordance with individualized care plans under RN supervision
- Monitor patient conditions, recognize changes, and communicate updates to the care team
- Coordinate care with interdisciplinary team members to ensure continuity and optimal outcomes
- Provide patient and family education on disease management, treatments, medications, and safety
- Accurately document care to support clinical, regulatory, and reimbursement requirements
- Participate in case conferences, team meetings, and ongoing care planning
- Promote patient independence and engagement through education and support
- Maintain compliance with organizational policies and quality/safety standards
- Active LPN license in the State of Illinois (or eligible)
- CPR Certification
- Pharmacology certification
- Minimum 2 years of Med/Surg clinical experience
- Valid driver’s license and reliable transportation
- Prior home health experience
- Familiarity with Medicare/Medicaid home health guidelines
- Strong clinical assessment and critical thinking skills
- Excellent communication and documentation abilities
- Ability to work independently and manage time effectively
- Comfortable using EMR systems and basic Microsoft Office tools
- Strong interpersonal skills with a patient-centered approach
- Travel throughout assigned geographic area to patient homes
- Ability to lift up to 35 lbs (with assistance/equipment as needed)
- Frequent standing, walking, bending, and patient assistance
- Work performed in patient homes and community settings
$23.60 – $35.30 per hour (based on experience)
