Job Description The Hospice Weekend On-Call RN provides compassionate, skilled nursing care and clinical support to hospice patients and their families during weekend hours. This position is responsible for responding to patient and caregiver needs, providing telephone triage, and making in-person visits as needed throughout the service area.
Responsibilities Provide hospice nursing support during the weekend on-call period
Respond to patient and caregiver calls and assess changes in condition
Make in-person visits as needed for symptom management and patient needs
Provide end-of-life care and support to patients and families
Complete hospice admissions, visits, and death visits as needed
Collaborate with physicians and the interdisciplinary team Address medication, equipment, and supply needs
Complete timely and accurate documentation in the EMRCommunicate important patient updates to the weekday hospice team Weekend Availability Must be available for the full weekend on-call period, Friday at 5:00 PM through Monday at 8:30 AM, including availability for telephone triage and in-person patient visits as needed.
Qualifications Current Illinois RN license
Hospice or home health experience preferred
Strong clinical assessment and communication skills
Ability to work independently and respond appropriately to urgent patient needs
Valid driver’s license, reliable transportation, and current auto insurance
Current CPR/BLS certification
Ability to travel throughout the assigned service area
