Growth and Community Partnerships Manager
- Care Diem Home care of Oak Brook, IL
Care Diem Home care of Oak Brook is seeking a Growth and Community Partnerships Manager to grow our Oak Brook and Downers Grove territory. This role combines outside sales, marketing and client engagement, requiring daily interactions with referral sources and prospective clients. The right candidate will be self-driven professional with proven success in building referral pipeline, generating leads, and converting them into long-term clients
Key Responsibilities- Develop and execute a strategic sales and marketing plan to meet growth goals.
- Conduct daily face-to-face calls and visits with referral sources, including hospitals case managers, skilled nursing/rehab facilities, assisted living communities , hospices, physicians, fiduciaries and other healthcare professionals.
- Build and maintain strong referral sources relationship by educating them on Care Diem's service, process and value
- Manage the lead pipeline daily using CRM tools, track all sales activities and conversions accurately.
- Conduct client assessment (in-home or virtual), ensuring all the Start of care documentation is completed prior to service initiation.
- Organize and represent Care Diem Home Care of Oak Brook at networking events, community events, and educational in-service to strengthen market presence.
- Keep management informed of progress, referral trends and competitor activities through weekly reporting.
- Stay informed on local home care competitors, pricing and promotional efforts to adjust strategies accordingly.
- Proven success in sales/business development role, preferably in home care, health care or senior care industry.
- Established referral source relationship in Downers Grove, Hinsdale, Oak brook or Naperville area strongly preferred.
- Comfortable with cold calling , networking and building new partnerships from the grounds up.
- Excellent interpersonal, presentation and communication skills with a professional demeanor.
- Strong organizational and time management abilities, capable of working independently with minimal supervision.
- Proficiency with CRM system and Microsoft Office.
- Bachelors degree in Business, Marketing or Healthcare related field preferred.
- Passion for senior care and client care.
- Highly personable, approachable, and able to build trust quickly.
- Self motivated, result-oriented and accountable for meeting referral and admission targets.
- Flexible to attend evening or weekend networking events as needed.
- Reliable transportation, ability to travel with the territory up to 90% of the time.
- Organized and able to manage multiple referrals and referral sources
