** Position Summary*
* The Speech Pathologist provides one‑to‑one, patient‑centered care using advanced technology and clinical expertise in the home setting. Care is delivered through a multidisciplinary approach, collaborating with physicians, nursing staff, social workers and home health aides.

Candidates must be highly motivated, detail‑oriented, organized, technologically proficient and comfortable working autonomously.

** Responsibilities*
* + Provide individualized speech therapy services in patients' homes

+ Collaborate with interdisciplinary team members to support comprehensive care plans

** Minimum Qualifications*
* + Meets educational and experience requirements for  
** Certificate of Clinical Competence in Speech Pathology (CCC‑SLP)
** from ASHA
** OR*
* + Meets educational requirements and is currently completing the supervised experience required for certification, registration or licensure in the State of Illinois (if applicable)
