Registered Nurse; RN - Home Health Case Manager - Southwest Suburbs

Position Summary

Registered Nurse – Home Health (Home Care Services) – Full time

Locations:
Oak Lawn, Burbank, Justice, Bridgeview, Hickory Hills, Willow Springs, Chicago Ridge.

40 hours per week. Days with weekend and holiday rotation. Must be able to work one summer and one winter holiday. Must rotate on an on‑call schedule.

Sign‑on bonus up to $10,000.00 subject to taxes. Bonus paid only when employed in the exact position at time of payout. Internal team members are not eligible.

• Provide home health and hospice services to homebound patients, establishing strong relationships with patients and caregivers.
• Assess, plan, implement, and evaluate nursing care for assigned patients in their homes.
• Set goals that demonstrate a holistic approach to patient and family care.
• Coordinate medical needs, act as the case manager, and utilize resources of the interdisciplinary team (e.g., social worker, CNA, MD).
• Document visits and care plans in point‑of‑care documentation and connect with the care team.
• Develop and maintain a therapeutic relationship throughout the healthcare continuum.
• Facilitate patients’ and families’ right to receive quality care.

Annual base salary: $74,672.00 – $, based on qualifications, skills, experience, and internal equity.
