Our pioneering Home Care Connect virtual care model helps patients avoid preventable ER visits and hospitalizations through advanced remote‑monitoring tools and 24/7 access to Virtual Care Center RNs.

The Speech Pathologist provides one‑to‑one, patient‑centered care using advanced technology and clinical expertise in the home setting. Care is delivered through a multidisciplinary approach, collaborating with physicians, nursing staff, social workers and home health aides.

Candidates must be highly motivated, detail‑oriented, organized, technologically proficient and comfortable working autonomously.

• Provide individualized speech therapy services in patients’ homes

• Meets educational and experience requirements for Certificate of Clinical Competence in Speech Pathology (CCC‑SLP) from ASHA
  OR
• Meets educational requirements and is currently completing the supervised experience required for certification, registration or licensure in the State of Illinois (if applicable)
