Role DescriptionThe Advanced Practice Nurse is a full-time, on-site role based at the Community Medical Clinic in Oak Grove, KY. This role is responsible for providing comprehensive primary and preventive care, including conducting patient assessments, performing physical examinations, diagnosing and managing acute and chronic conditions, and prescribing medications and treatments in accordance with clinical guidelines. The Advanced Practice Nurse will order and interpret diagnostic tests, develop individualized care plans, and collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated, patient-centered care.
Day-to-day duties include educating patients and families on health maintenance and disease prevention, documenting care in the electronic health record, participating in quality improvement initiatives, and supporting the health center’s mission of serving patients of all ages and backgrounds. The role may also involve participation in community outreach, health education programs, and adherence to all regulatory and organizational policies.
- Active Advanced Practice Registered Nurse (APRN) license in Kentucky, with national certification (e.g., FNP, ANP, or equivalent) and eligibility for prescriptive authority.
- Demonstrated clinical skills in primary and preventive care, including health assessments, diagnosis, treatment planning, and chronic disease management across the lifespan.
- Strong patient education and counseling abilities, with a focus on health promotion, disease prevention, and culturally responsive care for diverse populations.
- Proficiency in using electronic health records, maintaining accurate documentation, and following evidence-based clinical protocols and guidelines.
- Excellent interpersonal and communication skills, with the ability to collaborate effectively in an interdisciplinary team and build trust with patients and families.
- Commitment to serving underserved communities and working within a Federally Qualified Health Center setting, including familiarity with community health resources.
- Ability to manage multiple priorities, maintain professional judgment in fast-paced clinical environments, and adhere to regulatory, compliance, and safety standards.
- Graduate degree from an accredited nurse practitioner program; prior experience in family practice, community health, or primary care is preferred.
