The RN Chronic Care Management Coordinator is responsible for growth and maintenance of the care coordination program, including oversight and management of patients enrolled in care management services; ensuring completion of the annual wellness visit and follow up on all elements of the preventative plan of care; and completion of discussions of advance care planning with patients. Responsibilities are performed in collaboration with intra- and interdepartmental staff of the organization, outreach, disease management/care management, care coordination/health promotion, education/training, motivational support to patients, referral sources, and the community.
This position aims to improve the quality of life for patients enrolled in the program by supporting quality outcomes, smooth care transitions, coordination of care across the health continuum, and encouraging healthy lifestyle choices to reduce long-term effects of chronic illness. The role also represents the clinics across multiple constituents and oversees the financial performance of the program.
- Grow and maintain the care coordination program; oversee and manage patients enrolled in care management services.
- Ensure completion of the annual wellness visit and follow up on all elements of the preventative plan of care.
- Conduct discussions of advance care planning with patients.
- Collaborate with intra- and interdepartmental staff, outreach, disease management/care management, care coordination/health promotion, education/training, and motivational support to patients, referral sources, and the community.
- Support quality outcomes and smooth care transitions; coordinate care across the health continuum.
- Encourage healthy lifestyle choices to reduce long-term effects of chronic illness.
- Represent the clinics across multiple constituents and support the financial performance of the program.
- A minimum of one-year recent clinical professional nursing experience is required; 3-5 years recommended. Minimum of one-year recent home care experience preferred.
- Must be able to perform the essential functions of the job to serve patients of all ages.
- Knowledge/experience with Cross Tx CCM platform.
- 3 years clinical experience including working with the geriatric population preferred.
- Previous work experience with educating patients and patient goal setting preferred.
- Previous work experience in an autonomous position.
- Current licensure in the State of Washington as a Registered Nurse.
