Overview

We are seeking a full-time (32-40 hours per week) Nurse Practitioner for our Hilltop Center located in Hilltop, WV. The salary range is $101,750 - $117,000 per year.

Align Med Partners is a national medical group dedicated to improving health outcomes among residents of skilled nursing and senior living facilities.

Responsibilities

The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager and collaborates on-site with the Center Leadership Team to achieve center-specific goals. The provider works with the Medical Director and/or Attending Physician for patient-specific medical care. The provider supplies primary and urgent care services to nursing center residents in collaboration with other medical providers and works collaboratively with Center Leadership to deliver high-quality health care and compassion.

Qualifications
  • Graduate of an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner with a Master’s Degree in Nursing with concentration in Older Adult and/or Family Nurse Practitioner.
  • A minimum of two years experience working with adult or geriatric populations.
  • In states where practice allows, an unrestricted APRN professional license and DEA registration in each state of practice.
