Overview

Physical Therapist (PT) - Outpatient - Neuro Focus

This role reflects the Advocate Health Care compensation note: pay is based on skills and experience. Base pay range listed below and discussed with the recruiter.

Base pay range

$40.30/hr - $60.45/hr

Responsibilities
  • Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations using appropriate methods.
  • Establish a plan of care and implement individualized treatment based on evaluation results and functional goals; monitor outcomes and adjust the plan as needed; provide status updates to physicians and teammates.
  • Identify factors affecting patient physical and psychosocial comfort and address these needs; plan for and complete patient discharge, including home programs and patient/family education.
  • Coordinate discharge planning within the continuum of care and order equipment as indicated; communicate with stakeholders to achieve optimal patient care.
  • Complete required documentation per department policy and accreditation standards with appropriate co-signatures (evaluations, progress notes, reports, discharge summaries); record patient care and progress; manage daily statistics, billing, scheduling, and insurance authorizations; maintain productivity standards and adhere to billing and documentation timelines.
  • Maintain a clean and safe work area; perform manual techniques, patient positioning, transferring, and equipment use; apply knowledge of modality indications and contraindications; promote patient safety and infection control.
  • Participate in performance improvement activities to improve patient care outcomes and quality of care; manage caseload and coordinate coverage as needed.
  • Pursue professional growth through continuing education, department in-services, and literature; orient new staff to policies as requested; participate in coworker evaluations as requested.
  • May supervise physical therapy assistants and other staff in accordance with state and local requirements; delegate tasks while maintaining responsibility for patient care plans.
  • May supervise physical therapy students after one year of clinical experience.
Qualifications
  • Education
    Doctorate degree in Physical Therapy from an accredited program or equivalent education as required by graduation date; maintain continuing education requirements per state standards.
  • Certification / Registration / License
    Licensed Physical Therapist in the state of practice;
    Basic Life Support (BLS) for Healthcare Providers (AHA) within required timeframe unless exempted by department leadership.
  • Work Experience
    No experience required
  • Knowledge / Skills / Abilities
    Intermediate computer skills; excellent communication, interpersonal, and organizational abilities; strong decision-making and problem-solving skills; complete competency assessments per site standards; maintain annual educational requirements for hospital certifications as applicable; ability to provide age-appropriate care and interpret age-related data.
  • Physical Requirements And Working Conditions
    Ability to sit, stand, walk, lift, and squat throughout the workday; lift up to 50 lbs from floor to waist, up to 10 lbs from waist to overhead, and carry up to 40 lbs; push/pull with 30 lbs of force; perform a sliding transfer with assistance; repetitive use of hands for grasping, pushing/pulling, and fine manipulation; ability to wear protective clothing as needed;

    travel may be required; operate necessary equipment.
