Job Overview
Base pay range: $30.15/hr – $45.25/hr
$5000 sign‑on bonus for experienced Social Workers!
This position is a Full‑time Social Worker at Advocate Health Care, located in Oak Lawn, IL.
Responsibilities- Provide social work services to patients, families, and individuals including psychosocial assessment, screening, needs evaluation, intervention planning, discharge planning, and follow‑up.
- Participate as part of the multi‑disciplinary health care team to develop safe and timely coordination of care, including post‑acute placement, palliative/hospice services, medical equipment, home health, outpatient follow‑up, mental health resources, and community services.
- Advocate for patient involvement in the plan of care and coordinate interventions with other team members.
- Maintain current knowledge of community resources, legislation, regulations, and educate patients and families on these issues.
- Provide resources and links to post‑acute care, bereavement, ethical issues, end‑of‑life issues, domestic violence, guardianship, and other social matters.
- Communicate effectively with patients, family, health care team, and community agencies; facilitate resolution of issues impacting continuity of care (e.g., prior approvals, application requirements, transfer paperwork).
- Document activities according to department and program protocols or standards.
- Manage patient stay to optimize length of stay and ensure appropriate level of care; coordinate care to reduce readmissions.
- Assist with advance directive education and execution; participate in legal proceedings as necessary.
- Maintain professional standards and responsibilities in accordance with accreditation, state and NASW standards and code of ethics; complete required continuing education to sustain licensure.
- License/Registration/Certification: IL‑LSW eligible; LCSW preferred (issued by the state of Illinois) or Wisconsin Social Work Certification.
- Education:
Master’s degree in social work from a CSWE‑accredited school. - Experience:
1 year of social work experience in a healthcare setting. - Knowledge, Skills and Abilities:
demonstrated knowledge of social work principles and methods in a health care setting; strong time management, professional accountability, and documentation skills; effective communication and collaboration within multi‑disciplinary teams; adherence to safety principles of an accountable care organization.
- Position may require travel and exposure to weather and road conditions.
- Operate essential equipment for the role.
- Work in normal office and patient care environments; may be exposed to hazardous materials and life‑threatening diseases; personal protective equipment required as ordered.
- Weekend, holiday, and evening coverage per site requirement.
- Expect standing, sitting, speaking, reading/writing, and walking throughout the shift.
- Fast‑paced environment with established time constraints.
