Client Care Coordinator - Home Instead Oak Park
Location: Oak Park, IL
Status: Full-Time
Compensation: $55k - $60k per year

- Lead and conduct in-home care consultations with prospective clients and families
- Develop, implement, and maintain customized Care Plans
- Oversee ongoing client relationship management, ensuring satisfaction and high-quality care
- Conduct quality assurance visits at 24 hours, 7 days, 30 days, and quarterly
- Serve as the primary point of contact for client and family concerns, resolving issues quickly and compassionately
- Partner with scheduling to ensure strong caregiver-client matches
- Collaborate with recruitment and training teams to support caregiver readiness and performance
- Maintain detailed documentation in Well Sky and client files
- Participate in rotating on-call coverage with leadership team
- Bachelor’s degree in healthcare, social work, human services, or related field preferred
- Minimum 2 years of experience in client care management, case management, or home care leadership
- Excellent interpersonal and communication skills
Highly organized, proactive, and responsive - Proficiency in Microsoft Office and CRM platforms (e.g., Well Sky)
- Valid driver’s license, reliable transportation, and proof of insurance
