• Provide skilled nursing care to patients in a rehabilitation and skilled nursing setting
• Collaborate with the care team to develop and implement individualized care plans
• Monitor patient progress, report changes, and update care plans as needed
• Communicate effectively with patients, families, and healthcare providers to ensure continuity of care
• Participate in multi-disciplinary team meetings to discuss patient status and develop care plans
• Coordinate with interdisciplinary team members to ensure seamless care transitions
