Registered Nurse (RN) Hospice – Part Time

Graham Healthcare Group (Residential Home Health and Hospice) is seeking a compassionate Registered Nurse to provide hospice care and manage patient care plans in Traverse City and surrounding areas. This part‑time position requires a weekend/after‑hour rotation and 20 hours per week.

• Collaborate with hospice care team to manage and deliver patient care plans.
• Provide nursing care to patients across outpatient and home settings.
• Maintain accurate and timely charting using EMR software.
• Travel to patient locations (~20% of work time) and manage your own schedule.
• Meet critical deadlines for documentation and care coordination.

• Active RN license in the state of employment.
• Associate degree or higher from an accredited School of Nursing.
• Minimum two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to travel to patient locations.

• Experience with tablets, mobile phones and EMR software.
