Van Driver

Position: Van Driver-Hourly
Diversicare - 100 Elmhurst Drive - Responsibilities:
Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital;
Accompany each resident on all medical visits. Stay with resident(s) until visit is complete;
Observe the following guidelines at each visit: sign in resident upon entrance; present resident’s documents to office personnel;
Before leaving doctor’s office: obtain resident’s documents; obtain new prescriptions; ensure doctor’s order form is updated; obtain appointment cards if follow-up required;
After returning to facility, report to RCM and be prepared to answer questions concerning visit; perform other duties as required and assigned
