Key Responsibilities:

• Provide direct nursing care to patients within a transitional care unit, including assessment, planning, implementation, and evaluation.
• Collaborate effectively with healthcare team members to ensure comprehensive patient care.
• Maintain accurate and timely documentation consistent with facility policies and best practices.
• Adhere to established protocols for patient safety, including carrying out appropriate EKG testing.
• Manage patient ratios efficiently, typically 1:8-10, with the flexibility to float as needed.
• Support weekend coverage, approximately one weekend every other week or twice per month.
• Engage positively with patients and families to support rehabilitation and recovery goals.

Qualifications and Experience:

• Current RN licensure with demonstrated experience in Rehabilitation and Long-Term Acute Care nursing.
• Required certifications include BLS (AHA) and NIH Stroke Scale (NIHSS).
• Ability to pass EKG competency testing prior to assignment start.
• Comfortable working with patient ratios and floating within the unit as needed.
• Availability to work assigned weekend shifts.
• Willingness to comply with holiday coverage policies, with limited requests for time off during holidays.
