Full-Time with On-Call and Weekend Rotation

Compensation: $75,000 to $100,000 per year plus a $4,000 Sign-On Bonus

You ll identify fall risks before they become injuries, recognize medication issues before they become hospitalizations, educate caregivers, and help patients regain the confidence to safely remain at home.

• Provide skilled RN home health visits throughout the St. Paul territory
• Deliver wound care
  , medication management, injections, and disease-specific treatments
• Educate patients and caregivers on medications, diagnoses, and recovery plans
• Coordinate care with physicians, therapists, and interdisciplinary team members
• Monitor patient progress and communicate changes in condition
• Help patients remain safely at home while reducing avoidable hospitalizations

• Active Minnesota Registered Nurse (RN) license
• Experience in home health
  , hospital, rehabilitation, skilled nursing, or acute care
• Strong clinical judgment and assessment skills
• Excellent communication and patient education abilities
• Self-motivated and comfortable working independently
• Reliable transportation and willingness to travel throughout the St. Paul service area
