Home Health Care, Inc. is seeking an experienced Registered Nurse (RN) Case Manager for home health in the Greater Twin Cities Metro Area. This full-time role offers a Monday–Friday schedule with one weekend/month and one on-call shift per week, plus a $6,000 first-year bonus.
You will manage a home health caseload, perform OASIS assessments, and coordinate skilled nursing care. A Minnesota RN license and 3+ years of RN experience (home health preferred) are required.
