Beyond Hospice, a service of Episcopal Homes of Minnesota, is seeking a compassionate, experienced, and dedicated Registered Nurse Case Manager to provide exceptional end-of-life care to patients and their families. This role is ideal for an RN who is passionate about hospice care, values meaningful relationships, and is committed to helping patients live with comfort, dignity, and peace.

Pay & Schedule- Pay: $40.93-$56.66 per hour (based on experience)
- Schedule:
Monday
- Friday (8:00am-4:30pm) - * Must be able to work every 6th weekend.
- Complete comprehensive initial and ongoing nursing assessments to determine hospice needs.
- Develop, implement, and update individualized plans of care with the interdisciplinary team.
- Provide skilled nursing care focused on comfort, symptom management, and quality of life.
- Administer medications and treatments as ordered by the provider.
- Complete medication reconciliation and coordinate medication orders.
- Educate patients, families, and caregivers on medications, disease progression, comfort measures, and self-care.
- Provide emotional support and guidance to patients and families throughout the hospice journey.
- Coordinate medical equipment, supplies, and community resources.
- Serve as Case Manager for assigned patients and coordinate all aspects of care.
- Complete accurate clinical documentation and maintain compliance with hospice standards.
- Supervise and support Hospice Home Health Aides and Licensed Practical Nurses.
- Collaborate with physicians, social workers, chaplains, therapists, and other team members.
- Communicate patient condition changes and obtain provider orders as needed.
- Participate in interdisciplinary team meetings.
- Promote continuity of care through effective communication with patients, families, and healthcare partners.
- Participate in on-call rotation as assigned.
- Follow CMS, Medicare, and State of Minnesota hospice regulations.
- Participate in Quality Assessment and Performance Improvement (QAPI) initiatives.
- Maintain accurate documentation and uphold hospice quality standards.
- Build strong partnerships with contracted facilities and community providers.
- Educate facility staff on hospice philosophy and end-of-life care.
- Maintain professional competency through continuing education and training.
- Graduate of an accredited school of nursing.
- Current Minnesota Registered Nurse (RN) license.
- Excellent assessment, communication, critical thinking, and documentation skills.
- Valid driver's license, reliable transportation, and current auto insurance.
- Bachelor's degree in Nursing (BSN).
- Two (2) years of acute care nursing experience.
- Two (2) years of recent hospice or home health experience.
- Ability to lift, push, or pull up to 50 pounds.
- Frequent standing, walking, bending, reaching, and assisting with patient mobility.
- Ability to work in patients' homes, hospice settings, and community locations.
- Comfortable working in emotionally sensitive situations.
