Jobtailor is seeking an experienced RN to coordinate and facilitate high‑quality, cost‑effective care for ESRD and CKD patients in Oakland Park, FL. The role requires autonomy and collaboration with the healthcare team.
You will coordinate care among patients, families, physicians, and care teams, develop individualized care plans, and support patients through complex transitions. Five years of clinical nursing and renal experience are preferred;
Capella/DaVita software familiarity is expected.
