We are strengthening our East Bay team and are seeking a Physical Therapist who wants flexibility, independence, modern tools, and an organized home health structure behind them.

Compensation & Benefits- Salary:

- Flexible Scheduling
- Independently perform physical therapy evaluations and reassessments
- Develop and manage individualized plans of care
- Deliver skilled therapy focused on mobility, function, fall prevention, and independence
- Supervise and collaborate with PTAs as appropriate
- Communicate directly with physicians and the interdisciplinary team
- Educate patients and caregivers
- Complete documentation in Kinnser, supported by AI tools
- Serve as the primary PT voice in patient care planning
- Current California Physical Therapist license
- Valid California driver’s license
- Strong clinical judgment, organization, and time-management skills
- Home health experience preferred
- Strong outpatient, inpatient, or rehab PTs are encouraged to apply
