ScheduleMonday‑Friday 8am‑5pm (No weekends, No holidays, No on‑call)
Position OverviewThis position is responsible for the development and implementation of DaVita’s Patient Pathways Program through facilitating patient education, processing of timely dialysis placement referrals, and building relationships. It enhances relationships with local hospitals, doctors and other kidney care providers to enable patients to make informed decisions regarding their dialysis provider. The role also improves the patient education process by analyzing and sharing best practices with other department members.
Essential Duties & Responsibilities- Patient Education (35%):
Communicate with and treat patients with empathy, meet individually with patients requiring outpatient dialysis placement within the assigned region, provide educational materials on outpatient dialysis, modality options, vascular access, insurance, out‑of‑pocket costs, grant program options, and help patients select an outpatient dialysis center. - Outpatient Dialysis Placement Referral (30%):
Work with hospital case managers, social workers and physicians to place patients in outpatient dialysis clinics; arrange placements with local clinics; provide required documentation; assist with transitions from hospitals to dialysis facilities; place patients and document referrals per policy. - Relationship Building (15%):
Work closely with internal and external kidney‑care professionals, partner with regional directors and facility administrators to implement best practices for accepting patients from inpatient acute‑care settings; attend operational team meetings. - Reporting (10%):
Conduct operating reviews with management (at least weekly), record patient data in electronic tracking system, respond to reporting requests, assist in gathering patient details, attend hospital meetings and comply with audit, compliance and reporting requirements. - Innovation (5%):
Learn, develop and implement new ways for Patient Pathways to differentiate; inform leadership of customer needs; participate in special projects; educate local markets. - Other duties as assigned (5%).
100% of time is allocated to the duties listed above.
Education And Experience- 2 to 4 years vocational/technical school certificate degree.
- 4 to 6 years associate’s degree or apprenticeship.

The Wage Range for the role is $24.00 - $28.50/hour.
