We are seeking an experienced Physician Assistant (PA-C) to join our growing Mobile Primary Care team serving seniors in home‑based care, assisted living (ALF), skilled nursing facilities (SNF), and senior living communities.

This is not a traditional clinic role. This is community‑based, field‑based medicine focused on geriatrics, chronic disease management, dementia care, and value‑based care delivery.

If you are a Physician Assistant passionate about senior care, panel management, and independent practice, this role offers autonomy with strong multidisciplinary support.

What You’ll Do
  • Deliver Mobile Primary Care in home and senior living settings
  • Provide comprehensive geriatric assessments and chronic disease management
  • Manage hypertension, diabetes, CAD, CVA, dementia, and complex Medicare populations
  • Coordinate care with interdisciplinary teams across hospice, palliative, and behavioral health
  • Drive patient engagement, retention, and outcomes within a value‑based care framework
  • Practice independently while maintaining high‑quality documentation and follow‑through
What We’re Looking For
  • Active, unrestricted Physician Assistant (PA‑C) license
  • 1–2+ years of clinical experience in primary care or geriatrics
  • Experience in home‑based care, SNF, ALF, or senior living communities preferred
  • Strong clinical judgment and comfort practicing independently
  • Interest in leadership, panel growth, and clinical program development
  • Valid driver’s license (field‑based/mobile role)
  • Guaranteed base salary + productivity‑based compensation model
  • Sign‑on bridge available for full‑time providers
