Title: Weekend Home Health RN (Field)

Schedule: Full Time (24 visits per week with benefits) or Part Time (18 visits per week).
* Non‑traditional schedules are available, i.e., Thursday‑Monday

Locations: Area #2 covers the following cities:
Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont AND Area #8 covers Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington

Paid Training: Must be able to train for 5 days Mon‑Fri 9am‑5:30pm in San Ramon (onsite)

The Home Health RN monitors a patient’s overall health and, in partnership with a primary care physician, develops an individualized plan of care for their patients. Each RN functions as a case manager and works independently to deliver quality healthcare. They provide customized care and patient education about disease management and offer specialized services such as Simple to Complex Wound Therapy, Post‑Surgical Care Management, and Ostomy Care Management.

Our internal operational support team handles medication reconciliation, DME orders, MD signatures, coding, lab follow‑up, and more, allowing clinicians to function independently and schedule visits around other commitments. The home care setting provides quality time with patients in the comfort of their own homes.

• Minimum 1 year RN experience, preferably in acute, SNF, Home Health or Hospice settings
• Must possess an active CA Registered Nursing License
• AA, BSN, or MSN education
• Reliable transportation
• Current CPR or BLS certification
