Our NP or PA will perform comprehensive health assessments, including but not limited to the following:
- Obtain history and perform physical examination
- Complete review of medicines and related instructions
- Review home safety and psychosocial needs
- Fulfill documentation needs for STAR and HCC measures
- Obtain and document all pertinent information in patient’s charts and case‑management notes on the same day as patient is seen; include medications, end‑of‑life preferences and “drivers” of preventable hospital admissions
- Instruct patients, caregivers, and family to call you prior to accessing medical care, subject to life‑threatening emergencies
- Maintain patient safety and confidentiality, using universal precautions in all patient care
- Prescribe/order procedures, treatments, and medications within scope of practice
- Develop a high level of trust and intimacy with patients and families
- CA Licensed Nurse Practitioner or Physician Assistant
- 1+ year of experience in primary care
- Experience in population health management is a plus
The base salary for this position is $140,000 – $155,000 per year. In addition, you will receive a robust benefits program, including:

Office-based work involving mainly sedentary desk tasks. Activities are limited to the workday. External hires must pass a background check and drug screening.
