American Traveler is seeking an experienced RN Case Manager for an inpatient acute care hospital position requiring a CA RN license and a minimum of 5 years of RN Case Management experience.

Job Details

• Inpatient acute care hospital setting, Case Management unit,

• Responsible for care coordination and care transitions across the acute care patient experience,

• Collaborates with physicians, utilization managers, medical social workers, and bedside RNs to manage patient progression and transitions,

• May include assignment to the Emergency Department to address complex clinical and social situations to avoid unnecessary admissions,

• Day shift, 5x8-hour shifts (8:00-16:30),

• Every other weekend required,

Job Requirements

• Active CA RN license required,

• Current BLS certification required,

• Minimum 5 years of RN Case Management experience required,

• Inpatient acute hospital Case Management experience is required,

• 2 supervisor references obtained within the past year required for consideration,

• Copies of all certifications required for consideration,

Additional Information

• Conducts initial and ongoing patient assessments, including psychosocial and financial screenings, to develop comprehensive care coordination plans,

• Performs utilization management including level of care reviews, length of stay monitoring, and collaboration with physicians on documentation and care decisions,

• Manages care transitions by formulating discharge and post-acute plans in collaboration with patients, families, physicians, payers, and community resources,

• Ensures compliance with CMS, Joint Commission, and other regulatory requirements related to transitions of care,

• Identifies and refers quality, risk management, and psychosocial concerns to appropriate team members,

• Candidates must reside more than 50 miles from the facility to qualify for travel pay rates,

• Former staff must have been away from the facility for at least 1 year before returning as a traveler,

• Only 1 holiday RTO request will be approved
