Center for Elders’ Independence (CEI) is a PACE organization delivering integrated health care to the elderly through an interdisciplinary team. The Nurse Practitioner, reporting to the CMO and supervised by a PCP, will provide medical services at CEI clinics as part of the IDT under the PACE model.
The NP will diagnose, treat, and manage acute and chronic conditions, coordinate care with the IDT, and participate in comprehensive care planning while adhering to CEI's protocols and guidelines.
