Comprehensive assessment conducts initial and ongoing assessments of patients medical, psychosocial, and discharge planning needs. Care plan development collaborates with the interdisciplinary team to develop, implement, and update individualized discharge and transition plans. Care coordination & transitions arranges post-acute services (SNF, home health, DME, hospice, etc.) and ensures safe, timely transitions across levels of care. Interdisciplinary communication facilitates communication among providers, patients, families, payors, and community resources to remove barriers to discharge.
Documentation & compliance completes timely, accurate documentation in accordance with regulatory requirements, organizational policy, and quality standards.

Travel Compliance: SSN required at time of submittal. DOB required at time of submittal. Certs required at time of submittal. Please see job description/unit description to find list of required certs depending on unit. RTO always needs facility approval. 1 year experience required, however please refer to job description for any exceptions made depending on the unit. Valid CA License (required at time of submission).
Radius rule: 75 miles. 2 supervisor references preferred. Candidates will float within hospital when needed (please refer to JD for specifics by specialty). No specific charting experience required. Background policy to be noted prior to submission so we can seek approval prior to offer. Any permanent employee must be gone for at least 6 months. Labs must be completed labs 48-72 hours from signing.
Modules:
Modules are non-billable, average 7.25 hrs. Modules are completed during orientation and factored in NBO. Modules are tracked through e-learning during orientation.
