Seeking a Nurse Practitioner to join a PACE (Program of All-Inclusive Care for the Elderly) interdisciplinary team serving an adult and elderly population (ages 55+).
Position Highlights:- Provide adult Primary Care in a value-based model
- Seeing an average of 5-10 patients/day in a clinic setting
- Manage acute and chronic conditions, conduct assessments, and contribute to team-based care plans
- CA NP license (FNP, AGNP, or ANP), Furnishing License, DEA, CPR
- Open to new grads; experience in geriatrics or family practice a plus
- Must have at least 1 year of recent Primary Care experience
